APPOINTMENT HOURS
Office Hours: Monday – Friday, 10:00 a.m. – 6:00 p.m. We see clients by appointment only. Same day and Saturday appointments may also be available.
PAYMENT
Payment is due at time service(s) are rendered. We accept all major credit cards and most Health Spending Accounts (with credit card logo).
CANCELLATIONS
Please call within 24-hours if you are unable to keep your scheduled appointment. This allows us to provide that time slot to another client.
ASSESSMENTS FOR MINORS
Assessments for Minors: Clients under the age of 18 must be accompanied by responsible adult.
SUPPLEMENT SAMPLES
We sometimes offer you samples to help you try out a new supplement or tea before you purchase it. Remember that samples are not a way to fulfill your regimen. We do not always have samples of your supplements. Please do not rely on samples for your nutritional plan.
DISSMAL
If you are “dismissed” from our practice, it means you can no longer schedule appointments, get nutritional supplements or consider us your practitioner.
Common reasons for dismissal:
Failure to keep appointments, frequent no-shows
Noncompliant (constantly not following practitioner’s instructions for health issues)
Abusive to staff
Failure to pay your bill
Dismissal Process: We will send a letter to your last known address, via certified mail, notifying you that you are being dismissed.
